Conducting effective ongoing transition assessment with students with disabilities is critically important to identifying students' goals for adult life and creating appropriate IEPs that include needed transition services that prepare students to achieve their goals for the future. This interactive session will explore how students, educators, family members, and others can incorporate person-centered approaches that will greatly enhance the effectiveness of the transition assessment process, and simultaneously strengthen the collaboration among all members of the IEP team and others who support the students.
